New Resource Highlight: Delaware Home Visiting Journey Map

We’re excited to introduce the newly created Delaware Home Visiting Journey Map! A helpful tool designed to support both the home visiting workforce and the families they serve.

This visual guide offers a simplified, easy-to-follow explanation of what a family might experience when enrolled in one of Delaware’s evidence-based home visiting programs. It’s a great resource to help explain the process to families and enhance communication for providers.

The Home Visiting Journey Map will soon be available for download in the Home Visiting Toolkit on the DE Thrives website, making it easy to share and integrate into your work. In addition, we’ve ordered hard-copy tabletop displays for our partners to use at tabling and outreach events, making this resource even more accessible in community settings.
