Empowering a Survivor to Thrive

When a Home Visitor reached out to Quality Insights about a client who had recently experienced domestic violence, Community Health Worker Stephanie Duncan quickly stepped in to ensure she received the support she needed. Together, they met via Zoom to coordinate care and create a safe, compassionate plan forward.
Through a Social Determinants of Health assessment, Stephanie identified several areas where support could make a difference. She connected the client to local domestic-violence shelters and financial-empowerment classes designed for survivors. With this assistance, the client entered a shelter, began working with Stand by Me on financial education, and soon secured stable housing. Her credit score began to rise as she learned new financial management skills.
As her confidence grew, the client shared her dream of becoming a homeowner. Stephanie linked her to housing-case-management services, first-time homebuyer programs, and Habitat for Humanity. She also began volunteering with a doula program and proudly graduated from their training program. Inspired to give back, she now plans to complete CHW training and become a certified interpreter to serve her Creole-speaking community.
This client’s journey is a powerful reminder of how holistic, person-centered support — across organizations — can help survivors rebuild their lives and inspire them to help others do the same.
