Delaware MIECHV-SSDOH Innovation Will Be Presented Nationally

In 2023, the Delaware Maternal, Infant, and Early Childhood Home Visiting (MIECHV) Structural and Social Determinants of Health (SSDOH) Innovation Project launched to strengthen referral linkages between evidence-based home visiting programs (HFA, PAT, NFP) and community health worker (CHW) agencies. The project involved bridging gaps between home visiting and community support systems, strengthening cross-sector collaboration, improving SSDOH response, and sustaining family engagement. This model offers a scalable framework for integrating SSDOH services across maternal-child health networks. Joint training and relationship-building sessions (Zoom + in-person) fostered understanding of each other’s services.
This project, which continues today, was presented at the MIECHV Annual Grantee Meeting in Washington, D.C. Our work was presented in conjunction with the states of Oklahoma and West Virginia, who have carried out similar projects. We were excited to share the successes of our program!
