Title V Maternal and Child Health (MCH) Grant

A key source of support for promoting and improving the health and well-being of the nation’s mothers and children — including children with special needs and their families. Funding comes from federal and state sources.

Delaware Healthy Mother & Infant Consortium

An organization created in 2004, with Governor-appointed members, The Delaware Healthy Mother & Infant Consortium (DHMIC) coordinates Delaware efforts to prevent infant mortality and improve the health of Delaware families. DHMIC makes recommendations to the Governor to decrease the death rate for moms and babies.

Quality improvement

Delaware Perinatal Quality Collaborative (DPQC)

DPQC fosters collaboration across the six birthing hospitals in Delaware to improve the quality of care for mothers and babies. It brings together the specific expertise of physicians, epidemiologists, nurses, nurse-midwives, quality improvement experts, and all other practitioners involved in perinatal health outcomes.

Delaware Child Psychiatry Access Program (DCPAP)

The Delaware Child Psychiatry Access Program (DCPAP) offers consultation, guidance, and resources with a child and adolescent psychiatrist or behavioral health team member.

Healthy Women, Healthy Babies Zones

This free program for expecting African American women in Delaware focuses on personal health, wellness, nutrition, mental health, and prenatal care.

School-Based Health Centers (SBHCs)

School-Based Health Centers were developed to respond to the individual needs of Delaware’s children. The new strategic plan addresses 13 goals to help children access the health care system for critical health and wellness services.

Family health

Home Visiting

Evidence-based home visiting is a free voluntary service for at-risk pregnant women and families with with children up to age five (5) years. Programs have demonstrated improvements in a wide range of maternal and child health outcomes. The services provided support positive parenting, improve maternal and child health outcomes, help prevent child abuse and neglect and aid young children with literacy, meeting developmental milestones, and school readiness.

Community Health Workers

A Community Health Worker (CHW) helps women to get and stay healthy by linking them to key resources to meet specific needs related to the Social Determinants of Health. CHWs complement a woman’s health care team by addressing food insecurity, transportation, housing needs, and much more.

Education

Peer Educators

Educate and empower the next generation of healthy men and women through the Delaware Peer Educator program! This program provides young adults with the knowledge of the importance of getting and staying healthy BEFORE considering becoming a parent, and gives them the tools to spread the message of preconception health to their peers.

Help Me Grow

The HMG system is a framework that brings together child-serving partners to improve early childhood services. Help Me Grow 2-1-1 is a one-stop shop that offers programs, services, and helpful information for parents-to-be and families.
