What does a community health champion look like? Do they need to be a doctor? Can they be a community health worker? Can a local clinic be a community health champion? The answer is yes to all! A community health champion is any person or organization that goes the extra mile to improve the health and well-being in your community and throughout Delaware. Make sure they get the recognition they deserve!

Help us nominate the next individual or organization to recognize for their contribution to promoting health equity and eliminating health disparity in Delaware. Both individuals and organizations that have made a significant impact are eligible to receive the 2019 Community Health Champion Award.
